Surgical Managements of Pancreas Head and Periampullary Cancer


Abstract
This clinical study was performed on 62 patients with pancreas head cancer and periampullary cancer at the department of surgery of Chung Nam national university hostpital from September 1988 August 1992.
@ES The results were as folows:
@EN 1) Prevalent age groups were the 6th decade & the 7th decade and male to female ratio was 1.6 : 1.
2) Among the 62 patients, 29 cases(46.8%) were pancreas head cancer, 13 cases(21%0 were ampulla of Vater cancer, duodenal cancer and distal C.B.D. cancer were 10 cases, respectively.
3) The frequent symptoms were abdominal pain962.9%), weight loss(48.4%), and general weakness(22.6%), frequent signs were jaundice(56.5%), palpable mass(27.4%), abdominal tenderness (25.8%).
4) Important laboratory findings were elevated in bilirubin(62.9%). GOT, GPT(58.1%), and alkaline phospatase(56.5%) level.
5) Resectability of the ampulla of Vater cancer, douodenal cancer, distal C.B.D. cancer pancreas head cancer were 76.9%, 50%, 30%, 27.6% respectively and overall resectability was 41.9%.
6) The most common postoperative complication was delayed gastric emptying but there were no relationship between the preoperative PTBD or presence of cholangitis and postoperative comlication.
7) 3 yar survival rates after pancreaticoduodenectomy for the cancer of ampulla of Vater, duodenum, pancreas head were 55.4%, 40%, 33% respectively.
